PAN Correction – Required Documents
For PAN Card Father Name Correction, the applicant should submit the PAN CR-01 Form along with a copy of the PAN Card and the required supporting documents.
PAN CR-01 FORM + PAN CARD COPY + SUPPORTING DOCUMENTS
Important:
All supporting documents should contain correct and matching
applicant information wherever applicable.
1. Father’s Name Proof
Any one of the following documents:
- Driving Licence — Father’s Name should be mentioned
- Passport — Father’s Name should be mentioned
- Voter ID — Father’s Name should be mentioned
- Aadhaar Card — Father’s Name should be mentioned
- Ration Card — Father’s Name should be mentioned
- Certificate of Identity (Original) — Signed by an MP, MLA, Municipal Councilor or Gazetted Officer
Note:
For Father’s Name Correction, the submitted supporting document
should clearly mention the applicant’s father’s name.
2. Address Proof
Any one of the following documents:
- Aadhaar Card
- Voter ID
- Driving Licence
- Passport
- Post Office Passbook with the applicant’s address
3. Date of Birth (DOB) Proof
Any one of the following documents:
- Aadhaar Card
- Elector’s Photo Identity Card
- Driving Licence
- Passport
- Matriculation Certificate or Mark Sheet issued by a recognized board
- Birth Certificate issued by the Municipal Authority or any authorized office
- Photo Identity Card issued by the Central Government, State Government or PSU
- Domicile Certificate
- Central Government Health Service Scheme (CGHS) Photo Card
- Ex-Servicemen Contributory Health Scheme (ECHS) Photo Card
- Pension Payment Order
- Marriage Certificate issued by the Registrar of Marriages
- Affidavit sworn before a Magistrate stating the Date of Birth
DOB Proof Important:
The document submitted as Date of Birth proof should clearly
establish the applicant’s Date of Birth.
